・Those who visit for the first time, those who visit the medical department for the first time, and those who visited the hospital and it’s been a year or more since the last visit
Please submit your patient registration card and health insurance certificate together to the general reception "2. First visit / Follow-up visit reception".
・Those who make an appointment for a medical consultation / those who visit a medical department that was visited within a year
Please follow the instructions on the display of the follow-up visit reception machine.
・Those who have a letter of reference
Please submit the referral patient appointment slip, letter of reference, health insurance certificate etc. to the general reception "3. Referral reception".

If you have any medications, please take an out-of-hospital prescription.

If you need to change or cancel an outpatient appointment for unavoidable reasons, please call respective department by the day of the appointment.

- The order of treatment may be changed depending on the patient's condition.In mild cases, waiting time may be longer.

●Expenses for medical treatment without a letter of reference (Additional Fee for a First-time / Second-time Patient without a Referral )
If you visit this hospital without a letter of reference from another medical institution, you will be required to pay the following fees as additional fee to the medical expenses for respective medical department you visited according to the national system.

⑦Those who receive medical consultations due to work-related accidents, accidents in line of duty, traffic accidents, childbirth, and medical checkups
